Sam goes back to the floor and makes an error, while Iain's disappointment in his sister leads to tragic consequences.
The first day of Cal's planned strike arrives, and while he and his allies set up their picket line, Lily struggles to decide where her loyalties lie.
Inside the hospital, the industrial action causes trouble for Sam, who resolves to make up for staff shortages by manning the ED himself, despite his lack of experience.
Meanwhile, Iain's pride over Gemma's recent achievements is dashed when he realises she was the one who leaked lurid pictures of Alicia online, and a hard-line racist is admitted after being injured in a pub brawl.
The legendary Dame Siân Phillips guest stars.
Recurring character, Hugo Bonning, makes another appearance. He's played by Billy Angel.

Mrs Brown's Boys creator, writer and star, Brendan O'Carroll, dons Agnes' famous cardigan and curlers once again, throwing open the doors of the larger-than-life Irish matriarch's home to host a Saturday night chat show, filmed in front of a live studio audience.
She begins by inviting actor Pamela Anderson, tennis coach Judy Murray and entertainments manager and TV personality Louis Walsh, among others to join her in the sitting room.

The mammy of BBC Comedy, Mrs. Brown, is inviting the nation round to her house for a brand new Saturday night TV show, All Round to Mrs. Brown’s.
The hugely popular Mrs. Brown’s Boys cemented its status as the nation’s favourite sitcom last July, with its live episode airing on a Saturday night reaching over 11 million viewers and winning the Radio Times Most Popular 21st Century Sitcom poll.
Now BBC Entertainment and BBC One have commissioned a new Saturday night series with Agnes Brown and all the Mrs. Brown’s Boys favourites.
Agnes is about to open the door of her very familiar family home, as mammy makes mischief with celebrity guests, surprise audience shenanigans and outrageous stunts, all in front of a live studio audience… what could possibly go wrong?!
Mrs. Brown’s Boys creator, writer and star Brendan O’Carroll says: "The entire cast is excited by this. I think Agnes may be worried that she'll need a bigger kettle to make tea for everyone that's coming round!"
All Round to Mrs. Brown's is to be produced by Hungry Bear Media in conjunction with O’Carroll’s production company, BOC-Pix. It was commissioned by Charlotte Moore, Director, BBC Content; Kate Phillips, Controller Entertainment Commissioning; and Alan Tyler, Commissioning Editor, BBC Entertainment.
Charlotte Moore, Director of BBC Content says: “Bringing one of our biggest comedy stars Mrs. Brown to Saturday nights in 2017 with a new entertainment show is going to be full of fun and mischief and totally unpredictable, I can’t wait!”
Brendan added: "I have been waiting some time to work with Dan Baldwin and Hungry Bear. It is so exciting now to team up for this. Similarly one of the things that everyone at BOC-Pix enjoys about working with the BBC is that we are always given the encouragement to explore new ideas - like we did with the live episode of Mrs. Browns Boys in July.”
Managing Director of Hungry Bear Media, Dan Baldwin, says: “It’s a genuine pleasure to team up with Brendan, Jenny and the entire Mrs. Brown cast. They are brilliantly funny and we hope to take that magical humour from the sitcom and transfer it into a truly outstanding entertainment show for the whole country.”
It will be executive produced by Dan Baldwin and Andy Price for Hungry Bear Media, and executive produced by Fiona Gibney for BOC-Pix. The series producer will be Toby Brack. The series will transmit later this year.
